AZSC is a program of not-for-profit Hospice of the Valley, serving central Arizona since 1977. Unlike other palliative care programs operated by multistate corporations, AZSC is based locally, hires locally and has longstanding relationships with our community.
Care is provided through partnerships with contracted health insurance plans, and we encourage you to contact us if you need assistance with symptom management, coordination of care or access to health-support services. Even if the insurance provider isn’t a contracted partner, we have other supportive care programs that can help.

We want to improve your quality of life, whether it’s ensuring a smooth transition back home after a hospital stay or managing your condition to avoid emergency room visits and hospitalizations.
